It is a mix of heart rate and the gyroscopes measuring how much your hands are shaking. If there is something where you can count caffeine intake, it takes that into account too

They basically use heart rate and movement, which you can tell is very inaccurate, which is why a lot of the premium watches don’t have functions like that on default, because of how inaccurate it is.

Cheaper manufacturers like to pack their products with a lot of features to claim it can do a lot, while most of it is very unscientific or untested/non certified
